While those who are involved in DIP-IN know we are so much more than our outcomes on paper, we do look at measures of the project to assess how well we are working to accomplish our aims, and further the wellbeing of the DIP-IN communities.

With that in mind, as we close in on the end of the year, here are some of the impact numbers we have seen from the start of the project to now:

  • Resident steering committees in the three DIP-IN communities have funded 27 projects to build community capacity, improve quality of life, and support a culture of health in their communities.
  • Community Health Workers (CHWs) based at Eskenazi Health have worked with 540 clients who have experienced challenges managing their diabetes. These Eskenazi CHWs also work to assist clients to address social determinants of health that are barriers to them managing their personal health. Our evaluation has shown that clients who worked with the CHWs have significantly greater improvements in their hemoglobin A1c levels (a marker of glucose control) compared to clients who do not work with a CHW.
  • Neighborhood CHWs based out of local community organizations have had more than 14,000 encounters as part of community outreach events to raise awareness about risk factors for diabetes and to encourage screening for those at high risk. These CHWs have also been involved in network-building with almost 300 organizations.
